For entrance into a BDN event, you may be required to sign a liability policy that states the following:

Liability Waiver

By signing this form, I assert that I have read, understood, and agree to the following statements. I understand that I will not be permitted to participate in event activities if I do not sign this form.

I understand and agree that in participating in any dance, there is a possibility of physical injury or death. I voluntarily agree, therefore, to assume all risks and responsibility for any such injury or accident, which might occur to me during any _______________________ Workshop weekend activities. I also exempt, release, and indemnify the organizers, volunteers and/or staff members from any and all liability claims, demands, or causes of action whatsoever from any damage, loss, injury, or death to me, or property which may arise out of or in connection with participation in any activities conducted by and associated with the _______________________ Workshop event. I further hereby voluntarily agree to waive my rights and that of my heirs and assigns to hold the organizers, volunteers and/or staff members liable for such damage, loss, injury, or death. I understand that I should be aware of my physical limitations and agree not to exceed them. I also understand that there will be flash photography and will adjust accordingly to that environment.

I understand that photos will be taken throughout the event, and these photos will be posted to social media and may be used for advertising of future Blues Dance Nashville events. I voluntarily consent to my photo being taken as part of the event.
